Forms for Tryouts
Please note: You must to fill out a copy of the "Gateway 2012 Tryout Registration and Waiver form" for each and every tryout that you attend. These forms must be signed by both the player and the player's parent or legal guardian.
In addition, for the South County VBC tryout, you must also fill out the Information form (available above in either Word or PDF format.)
